Appointments help us ensure that:

• Your Live Scan Request Form is accurate and complete before submission

• You receive personalized assistance without unnecessary waiting time

• Each applicant’s fingerprints are captured efficiently and securely

Each appointment is scheduled in a 30 minute time slot. However, most Live Scan sessions are completed in just 5–10 minutes, depending on fingerprint quality and system processing time.

You will need to bring:

• A valid form of identification (e.g., driver’s license, state ID, passport, or military ID)

• Your Live Scan Request Form (BCIA 8016), completed by your employer or requesting agency

• A credit or debit card, or exact cash, if your agency does not cover your fees

Your Live Scan Request Form (BCIA 8016) must be fully and accurately completed before your appointment.
We cannot accept blank or handwritten forms. You should only fill out the personal information section (middle portion of the form).

Your employer or requesting agency is responsible for ensuring:

• The form is the correct version (BCIA 8016)

• The employer/agency information section is properly completed

• All required codes and identifiers are accurate

This ensures that your fingerprints are correctly routed to the appropriate agency.
We are not responsible for errors or delays resulting from incorrect or incomplete information on your form.

Your form must include:

• A valid ORI Number (assigned to the requesting agency)

• The Authorized Applicant Type

• A Working Title or description of the position

• For care facilities, a Facility License Number must be entered under the OCA Number field, and the employer’s address must be listed. Please follow the provided instructions for care facilities that is included in your packet. They are very specific to the type of facility.

To ensure your Live Scan appointment goes smoothly and your prints are accepted on the first submission, please follow these preparation tips:

Before your appointment:

• Make sure your Request for Live Scan (BCIA 8016) form is fully completed by your employer or agency.

• Bring a valid photo ID (driver’s license, state ID, or passport).

• If your agency does not cover fees, bring exact cash or a credit/debit card for payment.

On the day of your appointment:

• Clean and dry your hands: oils, lotions, or dirt can affect fingerprint quality.

• Avoid cuts, bandages, or injuries on fingertips whenever possible.

• If injured, bandaged, or sick/ill we will reschedule.

Appointments usually take 5–10 minutes when prints are clear and the form is correctly completed.

All forms of identification must be valid and cannot be expired. 

If your Driver's License is expired and you have a temp from the DMV, please bring the expired physical ID as well as your temporary ID paperwork and we can process your Livescan.

Acceptable Primary forms of photo identification include any of the following

• State-issued driver’s license
• State-issued photo ID
• U.S. Passport or U. S Passport Card
• Uniformed Services Identification Card
• Foreign Passport with Appropriate Immigration Document(s) (e.g., Form I-94)
• USCIS – Permanent Resident Card (I-551)
• USCIS – Employment Authorization Card (I-766)

Secondary Forms of Identification

In the absence of a primary form of identification, an applicant shall provide at least two of the following forms of secondary identification AND at least two additional forms of the supporting documentation that follows:

• State Government Issued Certificate of Birth
• U.S. Tribal or Bureau of Indian Affairs Identification Card
• Native American tribal document
• Social Security Card
• Marriage Certificate (Government Certificate Issued)
• School ID with photograph (minimum age 7 years old, or transaction will be rejected by DOJ) *
• Certificate of Citizenship (N560)
• Replacement Certificate of Citizenship (N-561)
• Certificate of Naturalization (N550)
• Replacement Certificate of Naturalization (N-570)

Supporting Documentation for Secondary Forms of Identification

• Utility Bill (Address)
• Jurisdictional Voter Registration Card
• Vehicle Registration Card/Title
• Paycheck Stub with Name/Address*
• Jurisdictional Public Assistance Card
• Spouse/Parent Affidavit

Our rolling fee is $25, which covers the cost of providing the fingerprinting service. However, total Live Scan fees vary depending on the type of background check requested by the agency and any additional fees charged by the California Department of Justice (DOJ) and Federal Bureau of Investigation (FBI).

Payment is required at the time of your appointment.
A receipt will be provided for your records and for any potential reimbursement by your requesting agency.

If your Live Scan is for one of our local school districts, the cost is often covered by the district, meaning you may not owe anything at your appointment. Districts that have you cover fees listed below:

• Lucerne Unified – Rolling fee only

• Upper Lake Unified – Rolling fee only

• Middletown Unified – All fees

• Konocti Unified - Nothing owed

• Kelseyville Unified - Nothing owed

• Lakeport Unified - Nothing owed

If your Live Scan form includes a Billing Number, this means your agency is covering the DOJ and/or FBI fees, and you will only need to pay our $25 rolling fee.

For Elder Care and Child Care facilities, individuals are typically responsible for paying all associated fees, as required by the state. Depending on the level of service (DOJ only, FBI only, or both), total costs can range approximately from $57 to $106.

The total Live Scan cost is made up of two parts: the rolling fee charged by the Live Scan provider and the background processing fees set by the DOJ and FBI. The DOJ determines these fees based on the purpose of the background check. For example, employment, licensing, or volunteer work; and whether both state and federal records are being searched. Because these fees are standardized by the DOJ and subject to change, exact totals may vary slightly between agencies or service types.

We accept exact cash only, as we are unable to provide change. Please bring the exact amount if you plan to pay in cash.

We also accept business checks issued only by the agency requesting your Live Scan. Personal checks are not accepted.

Because Live Scan fees vary depending on the type of request and agency, we recommend bringing a credit or debit card for the most convenient payment option.

Live Scan fees are determined by the California Department of Justice (DOJ) and may include both the DOJ processing fee and any additional rolling fees charged by the Live Scan provider. The total cost depends on the level of background check requested by the agency.

Live Scan results are typically transmitted to the California Department of Justice (DOJ) and/or Federal Bureau of Investigation (FBI) within 24 hours of your appointment.

Most results are processed and sent to the requesting agency within 3–7 business days. However, in some cases such as when a record requires manual review or additional verification, processing may take up to 30 days.

Please note that we do not receive or have access to your results. Results are sent directly to the agency listed on your Request for Live Scan (BCIA 8016) form.

You can check the status of your Live Scan submission online using the California DOJ’s Applicant Background Check Status tool at:

To check your status, you will need:

• Your ATI Number (found on your Live Scan receipt)

• Your Date of Birth

Please note that only the requesting agency receives the completed results. Our office cannot access or provide updates once your fingerprints have been submitted.

Live Scan results are sent directly to the requesting agency listed on your Request for Live Scan Form (BCIA 8016) and cannot be released by our office.

If you need to review your own background information, you can request a Record Review directly from the California Department of Justice. Instructions and forms for this process can be found on the DOJ’s website under “Request for Live Scan Service (Record Review)”.

This process allows individuals to obtain a personal copy of their background check, but it must be initiated separately from employment or licensing scans. Typically these cost $25 plus the Rolling Fee and are mailed directly to your mailing address which can increase the time it takes to receive results.

If your Live Scan submission is rejected by the DOJ or FBI due to fingerprint quality (smudges, unreadable prints, etc.), the agency will issue a reprint request.

In most cases, there is no additional DOJ or FBI fee for resubmission when the rejection is for image quality. Please bring the reprint request or rejection notice with you to your appointment.

The rolling fee is waived for resubmissions. We’ll gladly assist you in completing the reprint to ensure a successful submission.

If fingerprint quality remains very poor after the second submission, often due to worn ridges or other physical factors, the DOJ will typically conduct a “No-Print” background check. This allows your background check to be processed using name based verification so your Live Scan can be completed without further resubmissions.

We currently provide Live Scan fingerprinting only for the State of California.
If your background check or licensing agency is outside California, they may require ink fingerprint cards (FD-258) instead.

We do not offer traditional ink or card fingerprinting services, so you will need to contact another provider that offers manual fingerprinting for out of state or federal submissions.

No. Live Scan results cannot be shared, transferred, or reused between agencies.

Each Live Scan submission is unique to the requesting agency’s ORI code and application type listed on your Request for Live Scan (BCIA 8016) form.
This means that if you apply for a new job, license, or volunteer position; even within the same field, a new Live Scan is required.

For privacy and security reasons, Live Scan results are sent directly to the agency requesting them and are not stored or accessible for future use.
